Verification Requirements

If you wish to make a request or complaint regarding your personal information or privacy rights, including a request under the California Consumer Privacy Act (“CCPA”), please complete the form below. Completing the form will allow ExteNet to ensure that the request is coming from you and to protect the security of your personal information. Please double check and confirm that the information entered below is accurate to prevent a delay in responding to your request. If you are an Authorized Agent, in addition to the information in the form, please provide the Consumer’s Power of Attorney authorizing you to act on the Consumer’s behalf or a signed, written authorization from the Consumer identifying you and authorizing you to access their personal information.

Description of CCPA Rights

Right to Know – You may request that ExteNet discloses to you the categories of personal information, categories of sources of person information, purposes for collecting information and categories of third parties with whom we share your personal information that we collected about you for the 12-month period preceding your request.
Right to Deletion – You have the right to request that ExteNet deletes personal information that we have collected from you and retained, subject to certain exceptions.
Right to Access – You may request that ExteNet disclose to you specific pieces of personal information that we collected about you for the 12-month period preceding your request. ExteNet may choose to mask or limit disclosure of sensitive personal information to protect the security of that data. Note that we cannot provide you with specific pieces of your Personal Information if the disclosure creates a substantial, articulable, and unreasonable risk to the security of your Personal Information. Additionally, we are prohibited by law from disclosing to you your Social Security number, driver’s license number or other government-issued identification number, financial account information, any health insurance or medical identification number, an account password, or security questions and answers.
Right to Opt-out of the Sale of Personal Information - ExteNet does not sell consumer personal information, nor do we share your information with nonaffiliated 3rd parties for their own marketing purposes or any purpose other than to provide service to ExteNet.
